Best Custom WP Plugin & Theme Development Companies

I'm looking for some top companies that can do anything with WordPress. These should be companies whose development team is comprised of both theme developers and plugin developers that also have great programming abilities. What well known, well respected companies exist that can handle custom theme development and custom plugin development needs besides Crowd Favorite and Automattic?

We have several plugins in the WordPress repository and have created tons of custom themes for our clients.
