    This looks like a custom coded system. If you have the budget you can get it done. Post it on sites where there are wp developers and coders.

    You can also get it done by using an e-commerce plugin. There are many but I'd personally go with the woo themes woocommerce plugin: http://www.woothemes.com/woocommerce/

    You may also want to check gravity forms: http://www.gravityforms.com/ Although it started as a contact form plugin it can handle order forms as well.
